Spay/Neuter Assistance Application

J. Kirk Foundation will reimburse low cost spay/neuter surgical costs at the following rates:               

Dog Neuter 

Under 25lbs - $70    

26 to 50lbs - $80    

51 to 80lbs - $90    

81 to 100lbs - $100   

Over 101lbs - $120 

Dog Spay 

Under 25lbs - $80  

26 to 50lbs - $90  

51 to 80lbs - $100  

81 to 100lbs - $115 

Over 101lbs - $155  

Cat Neuter - $35  

Cat Spay - $45  

Higher fees or additional costs will not be reimbursed and are the responsibility of the pet owner.

J. Kirk Foundation will reimburse low income pet owners for low cost Spay or Neuter surgery for two pets per owner/per address, per year.

For consideration of financial assistance for Spay/Neuter for your pet, please complete the following information. All applicants must mail or email a copy of the invoices or receipts and proof of residence to J. Kirk Foundation within 14 days of sending this request.   

Not all applications will be approved for payment.
